Completed application form (HS / MS). Be sure to answer all applicable questions and sign the student's and parents' commitments on page 4; provide a copy of your most recent achievement test results (ISTEP + or other), your most recent report card, and immunization records from your previous school; a completed Tuition Payment Preference Form, two Personal Reference for Admission Forms, and the Permission for Treatment Form (which must be signed in the presence of a notary public). a non-refundable $35 application fee. If applying after June 1 for the upcoming school year, must also include a $300 down-payment.
