To apply to Bethany Christian Schools, please submit the items listed below plus a completed application. You may fill out the application online using our high school form or middle school form. If you prefer to fill out a written application and mail it to us, you may download the high school form (pdf) or the middle school form (pdf) (you may also download the forms listed below by clicking on the desired form):

• A photo of the student
• A copy of student’s most recent achievement test results (ISTEP+ or other).
• A copy of student’s most recent report card.
• Tuition Payment Preference form.
  
• Two Personal Reference for Admission forms (salmon) to your chosen references. (download a pdf copy)
• Permission for Treatment and Travel form—must be signed in the presence of a notary public.
• Immunization records from previous school.
• A non-refundable $35 application fee.
• A $300 down-payment if applying after June 1.
